Maggie Sottero Fiorella wedding dress in Pearl (off-white), Style A3325SS.
Size 8 dress was taken in and hemmed to fit me - I am 5'4" and wore 4" heels on my wedding day, approximately 125lbs, 27" waist and wear sizes 2-4 (Jcrew, Banana Republic) or 4-6 (Theory, Lululemon). A bustle and cups were added by my seamstress (I'm a 34B) - the cups can probably be removed if not needed.
